You get an HDMI splitter, or mirror your monitor from a second output on your GPU to send your monitor signal to the second computer into the capture card, where you can import the captured image just like a webcam and include it in your OBS scene. If you want to use a capture card to offload encoding and rendering from the computer that you game on, then you have to get a second computer to do the rendering (GPU) and encoding (CPU). If you have a Live Gamer HD and OBS Multiplatform or Xsplit, then yes, you can use the hardware encoder to offload encoding from your CPU (not GPU), but the encoder is a pile of garbage, and it prevents you from using the capture card to actually capture anything.
